PERFORMANCE WORK STATEMENT (PWS)FORMINOR CARE PHYSICIAN ASSISTANTSection One.1. Description of Services.1.1. Requirements. The Contractor shall provide personal services for Credentialed Positions referred to as the Health Care Provider (RCP) at San Antonio Texas Department of Defense Military Treatment Facilities (MTF). The HCP shall perform services and support compatible with the medical facility's operating capacity and equipment.1.1.1. All services shall be provided in accordance with established standards,-principles and ethics of the profession and applicable professional specialty organizations, The Joint Commission standards, Accreditation Association for Ambulatory Health Care, applicable Department of Defense (DoD), military specific regulations, directives, and policies, and MTF specific policies and procedures. Contract health care providers shall give the highest regard to patient dignity and observe the precepts of the American Hospital Association's Bill of Rights for Patients.1.3. Specific Duties. The HCPs duties shall include, but are not limited to: 1.3.2. PA Minor Care:1.3.2.1. The HCP shall provide primary or specialty medical care with the cognizance or supervision of a physician. Provides services by collecting data required for diagnosis and performing procedures and services. Obtains and evaluates medical histories, performs physical examinations, performs therapeutic and diagnostic procedures, and orders and interpret laboratory studies, radiographs, electrocardiograms, and other special examinations. Collects specimens for pathologic examination. Analyzes and interprets data, formulates problem lists, and establishes plans for solution of clinical problems. Counsels patients on medical problems, use of drugs, expected effects of treatment, family planning, child care, diet, and other therapeutic health maintenance matters. Makes entries in patient's permanent medical records. In emergency situations, administerslifesaving procedures and medications.1.3.2.2. The HCP shall be responsible for the coordination of patient care with other members of health care team. Initiates consultation requests to other health professionals to provide more comprehensive patient management. Accomplishes narrative summaries and other documentation.1.3.2.3. The HCP shall be responsible for providing guidance and/or consult other personnel assigned to health care team on preventive medicine programs and other appropriate medical activities and functions.1.3.2.4. The HCP shall be responsible for conducting 10 peer reviews on a monthly basis within 14 calendar days of assignment.1.4. General information.1.4.2.2. PA Minor Care: Routine services shall be performed in the MTF between the hours of 0700 through 2300 (Monday through Sunday).1.4.3. Absences. The Contractor shall provide qualified and trained personnel for the required hours of performance. The contractor shall ensure that scheduled and unscheduled absences do not interrupt service performance.1.4.3.1. Unscheduled absences shall be reported to COR no later than two (2) hours prior to the start of shift by the contractor.1.4.4. Staffing The Contractor shall provide a qualified and trained HCP to cover the required hours of performance. The Contractor shall staff HCP to meet the requirements in this PWS and ensure continuity of performance. If the HCP is absent for more than ten (10) working days, the Contractor shall initiate replacement action immediately.1.4.5. Inclement weather. In the event of unplanned closure of the facility due to natural disasters, military emergency, or severe weather, contract employees will be treated (for leave and pay purposes) in the same manner as DoD civilian employees.1.4.5.1. HCP on duty at the onset of inclement weather (as declared by local base officials) shall remain on duty until the end of their scheduled shift unless otherwise directed by government officials.1.4.5.2 In the event that HCP are on duty and cannot leave the installation due to inclement weather (as declared by local base authorities), the Contractor will be allowed to invoice for hours if the government determines HCP are necessary for MTF operation and instructs the contractor accordingly.1.4.6. The following is a list of legal federal Holidays: New Year's Day, January 1st; Martin Luther King's Birthday, 3rd Monday in January; President's, Day, 3rd Monday in February; Memorial Day, last Monday in May; Independence Day, July 4th; Labor Day, 1st Monday I September; Columbus Day, 2nd Monday in October; Veteran's Day, November 11th; Thanksgiving Day, 4th Thursday in November; Christmas Day, December 25th. (Minor Care Physician Assistants may be required to work some holidays as directed by the department)1.4.6.1. Family/Down Days. Services may be required on holidays to include any Family Day/Down Day. Typically, a "Family Day/Down Day" is associated with, but not limited to, the following holidays: New Year's Day, July 4th, Thanksgiving, and Christmas. The Contractor shall not bill the Government for a Family Day/Down Day unless the HCP is required to report to work.1.4.7. Performance of services during crisis declared by the National Command Authority or Overseas Combatant Commander- with the exception of the Minor Care PA, services under this contract have been determined not to be essential for performance during crisis is the Women's Health Nurse Practitioner.1.5. The HCP shall meet the following qualification requirements:1.5.1. The HCP shall meet the following education requirements: The HCPs education requirements shall include, but are not limited to:1.5.1.3. PA Minor Care: HCP shall graduate from a Physician Assistant Training Program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or its predecessors, and acceptable to the Surgeon General, USAF, is mandatory.1.5.2. Continuing Medical Education (CME). The HCP shall keep their license/registration/certification current. Required continuing education for CME shall be at no additional cost to the Government. The HCP shall provide appropriate CME documentation to the COR or designated government representatives within one (1) month after all CME accredited events.1.5.3. License and Certification Verification. The HCP/Contractor must possess and maintain an active current, valid, and unrestricted license from a US jurisdiction before practicing independently within_ the defined scope of practice for their specialty.1.5.3.2. PA Minor Care: Possess a current National Commission on Certification of Physician. Assistants (NCCPA) certification. 1.5.4. The HCP shall have and maintain a current certification and Life Support certification as follows:1.5.4.2. PA Minor Care: BLS, ACLS1.5.5. The Contractor shall perform all primary and annual licensure or certification document verifications for the HCP to include all replacement HCPs. The Contractor is responsible for insuring the HCP maintains these certifications in a current status throughout the life of this contract. Verifications shall be submitted to the government at least ten (10) working days prior to the proposed backup/replacement beginning work in accordance with attachment one (1).1.5.5.1. The Contractor shall confirm that the clinical licenses or certifications have not been suspended or revoked by any health care facility, public or private, anywhere in the world. This does not preclude any HCPs whose licensure or certification, although originally suspended or revoked by a health care facility, was subsequently fully reinstated by the same.1.5.5.2. Health care professionals who fail to maintain a license, registration or certification will be removed from independent practice and privileged providers will be placed in abeyance. If time exceeds 30 days of abeyance, the provider will be placed in summary suspension in accordance with (TAW) AFI 44-119, paragraph 9.16. until the license, registration or certification is renewed. Issuing agency grace periods are not honored by the AFMS when renewing a license, certification, or registration.1.5.6. The HCP shall complete the Initial profile package requirements (attachment one {1}). 1.5.7. The HCP shall meet the following experience requirements:1.5.7.2. PA Minor Care: HCP shall have a minimum of 24 months clinical practice experience within the past 36 months. If a graduate of the Interservice Physician Assistant Program within the past year experience can be waived.1.6. Conduct Requirements. The government reserves the right to restrict the performance on this contract by any individual who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the MTF and its population.1.6.1. The HCP shall not bill the patient for services rendered or accept compensation of any kind for patients treated, procedures performed, or any other actions performed, except under the terms and conditions of this contract.1.6.2. The HCP shall not advise, recommend, or suggest to persons eligible to receive care at Government expense that such persons should receive care at an outside agency or provider at any place other than as designated under this contract.1.6.3. The HCP shall not use Government facilities or other Government property for personal or other business not related to this contract.1.6.4. The HCP shall not respond to any media inquiries nor provide interviews, comments, or any other responses to the media. All inquiries or complaints from the media or other sources shall be immediately relayed to the CORCOR or designated government representatives.1.7. Security.1.7.1. The HOP shall follow all guidelines found in the Security Requirements for Solicitations and Contracts clause.1.7.2. Internal Operating Instructions (01). The HCP shall abide by the MTF's current °Is for internal circulation control, the protection of resources, and the regulated entry into Air Force-controlled areas during normal, simulated, and actual emergency operations. (See attachment 2)1.7.3. Criminal History Background Check (CHBC) (Children under the age of eighteen {18}). The Government will conduct criminal background checks and complete SF85P Questionnaire for National Security Positions for HCP's providing child care services under this contract. The Government will use the procedures set forth in the most current version of Public Law 100-235, DoD Directive 5200.28 Enclosure 6 paragraph E6.4.1.2, DoD Directive 5200.2-R, DODI 1402.5. Upon favorable completion of The NACI, the HCP may be selected to provide services to a minor child under Line-Of-Sight Supervision until the required background checks are completed. If the HCP has previously received a background check, proof of the check shall be provided or a new one obtained. (A new investigation is required if a break in service to the Department of Defense results in a time lapse of more than two (2) years). Line-Of-Sight Supervision is defined at Enclosure 5 of DODI 1402.5. The HCP may be employed under the contract pending completion of the background checks provided the activity uses line-of-sight supervision while minor children are in the care of the HCP. The HCP shall be within line-of-sight supervision of a staff person whose background check has been successfully completed or is in the presence of the child's parent or guardian. (Women's Health Nurse Practitioner)1.7.3.1. CHBCs must be repeated every five (5) years. CHBCs are to go back as far as possible, to age eighteen (18). Prior to age eighteen (18), the documents are not accessible.1.7.4. Reporting Requirements. The HCP shall comply with AFI 71-101, Volume 1, Criminal Investigations (Chapter 2, paragraph 2.7) and Volume-2, Protective Service Matters, (Paragraph 1.2). Contractor shall report to Security Forces any information or circumstances which may pose a threat to DOD or contractor personnel, resources, or DOD information.1.7.5. Removal of HCP. The government, through the CO, reserves the right to require immediate removal from contract performance on the installation or any government facility, any individual whose actions raise reasonable suspicion that patient care or services may be compromised in any way, or that pose a threat of harm to other contractor/government personnel or self Removal under other circumstances will be subsequent to, and at the direction of the CO only. 1.7.5.1. If a situation meriting removal occurs as outlined in the previous paragraph, the COR will contact the CO and the contractor's representative within twenty-four (24) hours. A meeting may be required with the CO, COR and Contractor representative to discuss further action.1.7.5.2. The CO will notify the Contractor if and when permanent removal is required. In the event of a disagreement between the Government and the Contractor, the decision of the CO will be final. During the period of time between the removal and the final decision of the CO, the Contractor agrees to provide backup/replacement HCP in accordance with the terms of this contract.1.7.5.3. The HCP may be required to submit to drug/alcohol testing. The government reserves the right to require temporary or permanent removal from contract performance any individual who refuses or fails testing.1.7.5.4. The HCP will ensure that its employees conduct themselves in a professional manner while on the installation and refrain from disruptive, offensive, or otherwise improper behavior that undermines order and discipline. The contracting officer may direct the contractor to remove from performance of this contract on this installation any contractor employee engaging in such misconduct.1.7.6. In/Out Processing. All personnel performing service under this contract shall be required to in-process through the 59 MDW Medical Logistics Contract Personnel Office upon reporting for duty. Upon termination or expiration of the contract, the HCP shall be required to out-process through the same office.1.7.7: Credentials Turn-In. Upon completion of performance, termination of the contract or termination of performance on this contract by any individual for any reason, the contractor shall out-process in accordance with paragraph 1.7.7.6. and turn in all CAC Cards, Base Identification Passes, Restricted Area Badges, keys and any other documents issued by the government, COR personnel, or Project Manager.1.8. Health Requirements and Immunizations. All HCP shall control and prevent disease in accordance with AFT 48-105 Surveillance Prevention and Control of Diseases and Conditions of Public Health or Military Significance, and the American Public Health Association Publication, Control of Communicable Diseases Manual, and the Centers for Disease Control and Prevention (CDC) publication, Morbidity and Mortality Weekly Report, Air Force Policy (as applicable) and its supplements. The Contractor shall provide proof of immunization or immunity from or screening for the following diseases according to Air Force guidelines equivalent to that provided to active duty armed forces: Hepatitis B, MMR (Measles, Mumps, and Rubella), Varicella, Tdap (Tetanus, Diphtheria, and Pertussis), Tuberculosis (TB), HIV (HCP who have direct patient care) and influenza for all personnel prior to contract performance. For the TB screening, the Contractor shall provide proof of negative TB 2-step skin testing IAW CDC recommendations outlined in attachment one (1) of this document. If an individual has a past positive TB skin test, the Contractor shall provide proof of a negative chest X-ray within twelve (12) months prior to personnel performing. The Government reserves the right to require more frequent Tuberculosis testing of personnel based on the facilities' annual TB risk assessment. Additional vaccines as they are made mandatory for health care workers may also be required in unusual circumstances.1.8.1. Medical Tests. The Contractor shall not obtain medical tests or procedures at the MTh, unless identified in Section three (3), the Contractor shall be responsible for the cost of any emergency medical services received.1.8.2. Influenza Immunization. The HCP shall receive the current influenza immunization, at contractor's expense, equivalent to that provided to active duty armed forces personnel unless contraindicated by allergy; as an example, to eggs as evidenced by hives, difficulty breathing, itching, and other symptoms of anaphylactic hypersensitivity and/or physician's diagnosis. At Government's discretion it can be accomplished as stated in paragraph 3.7.1. of the PWS.1.8.3. Hazardous Materials. This PWS does not call for any hazardous material to be provided by the Contractor or HCP.
