Physical Therapist – PT – Healthcare – Home CareVisiting Nurse Services of ConnecticutConnecticut – Fairfield, New Haven, and Litchfield Counties Visiting Nurse Services (VNS) of Connecticut is currently hiring for a Physical Therapists in the Trumbull, Norwalk, Bridgeport, Oxford and Torrington areas. This individual will be responsible for the evaluation, supervision and delivery of physical therapy services to agency patients in their place of residence.    This is performed in accordance with an established physician plan of care under the direction and supervision of the Director of Professional Services/Administrator while in compliance with all applicable federal, state and agency policies and procedures.  ***We are currently seeking both full-time, part-time and per visit licensed professional’s to serve as PT staff***Responsibilities:Evaluates patient’s level of function and developing an appropriate plan of care for patients in their home environment. Provides rehabilitative therapy including exercise, gait, balance and prosthetics training to help restore patients to their highest functioning levels of strength, range of motion and mobility. Ensures that Branch and Agency standards, policies, procedures and protocols, and goals and objectives are achieved. Delegates appropriate patient care responsibilities to PTA and Home Health Aides, patient, and family members in the care of the patient. Responsible for accurate completion of Oasis documentation.  Preferred Skills and Qualifications:Licensed Physical Therapist in the state of Connecticut 1+ years of experience as a Physical Therapist  (Home Care experience preferred) “Bringing HEART to Home Care” is much more than a slogan.  It is the essence of our caring philosophy and organizational culture.
