The Pharmacist is responsible for all activities to ensure safe and appropriate pharmaceutical usage in accordance with local, state and federal laws, regulations and guidelines. Collaborate with Clinical Operations team to develop and enhance services with emphasis on Medicare Part D, and Drug Therapy Management will also support Therapeutic teams. Bachelors in Science of Pharmacy, degree from an accredited College of Pharmacy, BCGP or BCPS, or PharmD a plus. 3 - 5 years of long term care consulting experience combined with knowledge of best practices in chronic care and geriatric issues needed.
