Critical Care Clinical Pharmacist job in Texas. Seeking Critical Clinical Pharmacist to work the day shift. Under the supervision of the Regional Clinical Coordinator and the Regional Director of Pharmacy will work directly with the health care team and patients to provide safe and effective pharmacy services in addition to the dispensing of medications, provides consultative services to medical, nursing and pharmacy staff. Ensures appropriate drug therapy, implements and monitors clinical protocols and guidelines, and enhances the delivery of evidence-based medicine. Other responsibilities include education, representation on medical center committees, and collaboration on clinical initiatives. Documents clinical activities and outcomes. Provides prescribed medications and compounds according to professional standards and practice. Performs at an advanced skills level. Prioritizes work demands. Participates in various professional education activities, which includes working with students from the local Colleges of Pharmacy and Pharmacy Practice Residents. Assists with development and mentoring of clinical staff pharmacist. EDUCATION: Graduate of accredited College of Pharmacy. EXPERIENCE: 2 years hospital clinical experience in Critical Care or 1 year hospital clinical experience in Critical Care and completion of an accredited Critical Care residency program. LICENSE/CERTIFICATION: Possession of current Texas State Board of Pharmacy License or eligible for reciprocity.
