Lakeside Health System is currently seeking an Registered Nurse or Registered Dietician with CDE to work as a Wellness Coordinator. Lakeside Healthcare System is located in Brockport, New York. We are a 181-bed healthcare system, serving a three-county area. In addition to Lakeside Memorial Hospital, we have an on-site child care facility, the Beikirch Care Center (a skilled nursing facility), a Family Wellness Center, and Urgent Care Centers in Spencerport and LeRoy. The wellness program coordinator assures that programs consistent with the mission of the Lakeside Health System/Family Wellness Program are made available for the Lakeside community and surrounding market area. The coordinator’s responsibilities may include, but are not limited to, the following: * Continually reviews and updates programs consistent with the Family Wellness Program mission. * Identifies community health education needs. * Oversees the Diabetes Self Management Education Program. * Determines needs of physicians on Health System staff and delivers classes supporting health education needs of the community. * Organizes the program calendar for promotion and marketing in a timely manner; works with internal/external resources for program promotion. * Maintains peer network and participates in appropriate area committees and councils that address wellness initiatives. * Provides direction to technical, professional, and secretarial staff utilized by the Family Wellness Program and completes their annual evaluations. * Directs and coordinates departmental personnel functions, i.e., interviewing, hiring, orientation, scheduling, promotions, and disciplinary action, as needed. * Demonstrates practices to keep all patient information protected and confidential according to HIPAA regulations. Applicants must meet the following criteria for consideration:* Current and valid NYS RN or RD License* CDE certification* CPR & BLS certification* Five years or more experience in acute care* Experience in health education is preferred.
