Home Health Organization in southern Texas is seeking an RN Case Manager. This is an in-house full time position, but the case manager will work in the field as needed. The adult census averages 142, and the range of care includes: wound care and wound vacs, trachs, full range of diabetic care and skilled nursing. The company is also in the process of implementing wound care chart review. The case manager will also review and resolve clinical billings issues. Qualifications include: strong leadership skills and at least one year recent experience in the home health industry; knowledge and proficiency in OASIS data collection and diagnosis coding is required; review of the Plan of Care and reviewing the Oasis information for chart checks.
