You have a BSN in Nursing and are licensed in the state of Connecticut.  You have professional certification in case management or clinical specialty is preferred.   Must have at least five (5) years of nursing experience in a medical surgical acute care setting.  You seek a weekend opportunity (8:00 am - 8:00 pm) with an organization that is fully committed to delivering high-quality, compassionate care.If the above applies to you, we would very much like to speak with you.  We seek candidates for the role of Care Manager.  In this role you will provide a link between provider and payer organizations, physicians and the community in the transition of patient care through the health care system.  You will monitor all admissions for the appropriateness of observation services or inpatient hospitalization, according to InterQual criteria.  You will utilize the nursing process, identify patients with discharge planning needs, and coordinate necessary services in the home or nursing facility setting.   You will coordinate patient care by using independent nursing judgment and working collaboratively with the interdisciplinary team.   Candidates must be willing to work every Saturday and Sunday from 8:00 am - 8:00 pm.  Must have at least five (5) years of nursing experience in a medical surgical acute care setting.  Preference will be given to candidates who have additional experience in case management, home care, and utilization review/management.  Must have good analytical, organizational and interpersonal skills as well as the ability to communicate effectively in English, both verbally and in writing.  Very competitive salary and full benefits for this weekend only opportunity.  Candidates who seek part-time day opportunities should feel free to inquire.  For more information call 941.739.1400 or submit your resume now by clicking on the web URL below.
