If the above describes you, we would like to get to know you.  We seek candidates for the role of Clinical Pharmacist Specialist.  In this role you will optimize medication use among ambulatory care patients leading to improved clinical outcomes while reducing/minimizing cost.  You will participate in patient care.  You will provide verbal and written consultations concerning drug therapy evaluations, as requested.  You will provide individualized patient education/evaluation regarding drug therapy as requested or identified by need.  You will document drug use patterns, detect and evaluate adverse drug effects, evaluate drug efficacy, therapeutic goals and indications, review for contraindications/precautions, the patient’s current status of health, and review drug profiles and patient’s medical record for drug-drug, drug-food, drug-laboratory and drug-disease state(s) potentials.
