St. Ann’s Community is seeking a full time Occupational Therapists for their Rochester, NY location. St. Ann’s Community is a not-for-profit organization providing a varied range of services and care levels designed to meet the individual needs of older adults. St. Ann’s offers several independent and retirement living options, short-term rehabilitation, transitional care, Adult Day Services, Assisted Living, specialized memory care and 24-hour skilled nursing care. The Occupational Therapist will report directly to the Occupational Therapy Coordinator. The primary purpose of this position is to provide high quality evaluation and treatment services for individuals requiring Occupational Therapy Services. This will include planning, organizing, developing, implementing, evaluating and directing Occupational Therapy, as well as its programs and activities, in accordance with the current applicable federal, state and local standards, rules, regulations and guidelines that govern long term, sub acute and outpatient care. Responsibilities for this position may include, but is not limited to, the following: * Assists in the supervision of volunteers, aides and assistants and per diem staff. Acts on behalf of the OT Coordinator in his/her absence, upon request. * Provide evaluation and treatment services for Patients, Residents, Participants and Outpatients of St. Ann’s Community as ordered by qualified medical practitioners. Consistently deliver high quality service, being responsive to internal and external customer needs, within the scope of practice as defined by the American Occupational Therapy Association and the New York State Department of Education. *Complete required documentation on time. Ensure that notes are informative and descriptive of education and care provided and of the resident’s response to care; seek renewal of orders as required; complete MDS/RAPs by due date; complete care plans; accurately complete billing daily, etc. Protect the confidentiality of these records. * Complete home assessments and Performance ADL assessments in a timely manner. *Model positive, proactive and team oriented behavior. * Make rounds on Nursing Units bimonthly to monitor resident performance and act as a resource for nursing personnel. * Plan, coordinate and implement in-service education as needed. The ideal candidate must possess a B.S. in Occupational Therapy from an accredited school. M.S. preferred. Two years experience as an Occupational Therapist preferred, with experience with adult and geriatric populations preferred. Requires a current NYS license/registration as an Occupational Therapist.
