Wesley Health Care Center is currently seeking an RN for a Part Time MDS/Rehab position. The Wesley Community’s distinctive continuum of care offers a unique balance of community living and care-giving that ensures quality of life at every stage of life. An integral part of the vibrant and diverse city of Saratoga Springs, New York, our beautiful campus is located on 36 acres and offers Adult Day Services, Assisted Living, Independent Living, Long Term Care, Outpatient Therapies and Short Term Rehabilitation. The MDS/Rehab Nurse is professional nurse who is responsible for completion of MDS schedules. Assuming assigned duties in absence of Resident Care Coordinator. The duties for this position may include, but are not limited to, the following: * Interpret the MDS regulations to members of the Comprehensive Care Team as needed. * Schedule all MDS completion dates and comprehensive care plan meetings excluding short term rehab “initial” care plans in absence of/as assigned by Resident Care Coordinator. * Assist with completion of MDS and care plans when necessary. * Schedule PPS Medicare/HMO MDS schedules and their RUG categories in absence of/or as assigned by Resident Care Coordinator. * Assisting in educating a new, and established staff in the MDS process, i.e. MDS completion, use of CAT’S and CAA’s, care planning. Applicants for this position MUST meet the following criteria for consideration: * MUST have a current and valid NYS RN Nursing License * MUST have comprehensive knowledge of and/or experience utilizing MDS 3.0. * MUST have at least 1 year of recent nursing experience in a geriatric setting. * MUST be able and available to work two 8-hour shifts and one 4-hour shift, totaling 20 hours, between Monday – Friday. * MUST be computer proficient, including knowledge of Microsoft Word and Outlook. Previous supervisory experience, previous knowledge of SOS software for tracking MDS, and knowledge of how to use the ADL tracking system called CareTracker are preferred.
