JOB DESCRIPTIONTitle: Director of Patient Care Status: Full time / ExemptDepartment: Patient Care Services Reports To: Executive DirectorPosition SummaryThe Director of Patient Care is responsible for overall administration of patient services including coordinating services with the interdisciplinary team of physician, registered nurses, social workers, ancillary professionals and nursing administration support staff. Oversee development and enforcement of policies, procedures and practices to ensure regulatory compliance. As a member of the leadership team, the incumbent is involved in overall agency planning, development of programs, and fiscal management. The Director of Patient Care represents HSLC to the medical community, other agencies and the general public. Accountability The Director of Patient Care is the direct supervisor for patient care staff (Social Workers, Registered Nurses, LVNs, Hospice Aides, Physical Therapist, and Dietician.) Also supervises patient support staff (the Admissions/Clinical Education Coordinator, Clinical Support Specialist and Patient Services Specialist.) The position reports directly to the Executive Director and works closely with the Medical Director. Essential Duties and ResponsibilitiesA. Leadership • Provides leadership for and participates in program development and overall agency planning, including development of policies, procedures and guidelines to fulfill agency objectives. Monitors compliance in areas of responsibility.• Keeps abreast of and monitors agency compliance with applicable state/federal regulations for licensure for hospice certification. • Communicates needs of nursing staff to Executive Director and participates in planning and budgeting. • Provides input to related administrative/computerized systems and monitors the processing of data relating to patient care. • Encourages understanding of clinical roles and coordination of services through participation in orientation for all hospice staff. • Acts as alternate Administrator in the absence of the Executive Director for matters related to patient care. • Serves as member of leadership team. • Assists in developing staff education programs for all clinical staff. • Participates in volunteer and other departmental and agency wide training as requested. • Develops and meets departmental goals and objectives as a part of the annual business plan. B. Director of Patient Care • Promotes professionalism and quality of care within the Patient Care Department. • Assures the competence of department personnel through leadership activities related to recruitment, interview, hiring, orientation, education, motivation, supervision, and evaluation. • Oversees delivery of patient care services and compliance with agency policies, procedures, mission, values, and goals. • In conjunction the HR Department, recruits, hires, orients and maintains adequate staffing that assures patient care services are available to patients and families 24/7. • Participates in activities relevant to professional services furnished, including the development of qualifications, assignment, supervision, and evaluation of contracted agency personnel. • Implements minimum weekly visit standards, visit performance standards, clinical documentation requirements and other clinical expectations. Documents and counsels individual clinicians related to compliance with standards monthly and as a part of the annual review process. • Demonstrated ability to assess and respond to the pain and symptom management needs of patients in home, and SNF settings and provide care instruction to the Hospice patient, caregiver and family. • Demonstrated ability to listen and respond non-judgmentally with respect to other’s beliefs and to communicate effectively verbally and in writing.C. Interdisciplinary Team • Working with the Medical Director, oversees the functioning of the interdisciplinary team (IDT), supervising and supporting members, including assisting with patient referrals/admissions as needed. • Acts as liaison with patients, families, physicians and caregivers. • Keeps abreast of issues regarding clinical documentation and monitors compliance according to state, federal and CHAP regulations. • Assures a reassessment of a client’s needs as performed by the appropriate health care professional as required: by compliance standards; when there is a significant change in the client’s condition; at a physician’s request; or after hospitalization. • Assures that all patients are timely reviewed and meet criteria/benchmarks for recertification for hospice services. Makes recommendations to the Medical Director. D. Performance Improvement • Oversees and coordinates with Director of Quality for continuous improvement by identifying and developing processes that will most benefit patients and their families. • Regularly evaluates the practice and documentation of staff, using findings to strategize opportunities for improvement. • Identifies and implements strategies to enhance efficiency and effectiveness of nursing, social work, and therapy services. E. General • Supports philosophy and objectives of HSLC. • Maintains professional appearance, behavior, attitude and integrity. • Keeps abreast of and abides by the policies and procedures of the agency. • Stays current with governmental and accreditation requirements affecting areas of responsibility. • Participates in appropriate meetings and activities, including Performance Improvement. • Exhibits good stewardship of resources, including self-care. • Reads materials and attends in-services, seminars or conventions as needed for professional growth. • Contributes to an atmosphere of excellence, mutual respect and caring. • Participates in other projects as assigned by the Executive Director. Minimum Qualifications:• Graduate of an accredited nursing program and current California Registered Nurse license. • Minimum three (3) years of supervision in a clinical setting.• Minimum of five (5) years of experience as a professional nurse in any of the following settings: hospice, home health agency, and/or hospital. • Ability to write and maintain proper documentation.• Strong computer knowledge pertaining to electronic medical records and MS Office products (Word, Excel, Outlook.)• Current California driver’s license, proof of acceptable automobile insurance coverage, and reliable transportation.
