CLINICAL PEDIATRIC PHARMACISTAn excellent pharmacist is needed for an outstanding opportunity near Fresno.Salary: $60.20 - $66.95 per Hour, with $12,000 sign-on bonus, $40,000 loan repayment and $5,000 relocation allowance.Education : Requires Registered Pharmacist license to practice in California.Qualifications : Pharm.D degree and a Pharmacy Pediatric Residency Certificate (ASHP Accredited program given priority) or equivalenthospital pharmacy experience (minimum 5 years).Duties : 1. Daily participation in multidisciplinary patient care rounds2. Protocol/process development to contain costs, improve pharmaceutical care, reduce hospital length of stay3. Daily medication profile review (types of interventions)a. Antimicrobial effectiveness/appropriatenessb. Pharmacokineticsc. Therapeutic consultationd. Drug allergy/contraindicatione. Dose adjustments (per renal/hepatic/age and/or weight specific)f. Order clarification/confirmationg. Therapeutic duplication minimizationh. Drug informationi. Lab/medication monitoringj. Formulary alternative recommendations when non-formulary agent requestedk. Cost containmentl. Target drug monitoring4. Intervention documentation/reporting5. Improve current competency assessment and training program to assist with pharmacist cross coverage capability6. Work with ISS to design customized pediatric formulary and improve computer system (safer, efficient, easier order entry process)7. Facilitate PCO development for new/current physician orders sets8. Streamline drug distribution process (safer/easier preparation and dispensing)9. Develop pediatric specific code blue and hyperalimentation competency assessment and training programs10. MUE as it relates to formulary management (e.g. xopenex)11. Formulary management prepare and present monographs for formulary addition requests and prepare education piece for nursing/pharmacy staff12. Implement processes to meet JCAHO medication management standards related to peds/nicu areas13. Assist in the development of a pediatrics hyperalimentation order set14. Peds/NICU syringe checkingCoverage area (s): Provide pharmacist coverage for the following patient care areas: medication order entry for new orders in the pediatrics and intermediate care nursery, labor and delivery.Hours: One Peds Pharmacist:Monday through Friday, 8 hour day shift. No weekend staffing requirement except in critical staffing situations. Two Peds Pharmacists:Variable 10 hour shifts. Every other weekend requirement.$12,000 sign on bonus for 2 year commitment paid in 3 increments (12 -18 - 24 months).
