We are seeking an RN Case Manager or LCSW to support our troops at the TBI Unit National Naval Medical Center, Bethesda, MDQualificationsRN Case Manager•        Education as follows:  Either (a), a Baccalaureate Degree in Nursing; or (b) a Diploma Graduate in Nursing; or (c) an Associate Degree in Nursing from an accredited university.•        Posses a current unrestricted license to practice as a registered nurse in one of the fifty states, the District of Columbia, the Commonwealth of Puerto Rico, Guam, or the U.S. Virgin Islands.•        Possess current certification in a minimum of one of the following:  CCM, CDMS, CRRN, COHN, ACCC, CRC, RN-NCM or CMC- OR -•        Possess a minimum of two (2) years full-time experience within the last three (3) years as a registered nurse providing case management and obtain one of the certifications in 4.b.(3) within 24 months of commencing service under this contract- OR -•        Possess a Masters degree in Nurse Case Management and obtain one of the certifications in b.3 above within 24 months of commencing service under this contract.•        Possess a valid driver’s license and a personal vehicle for use under the contract.LCSW•        Master’s degree in Social Work from a program accredited by the council on Social Work Education (CSWE).•        A minimum of 2 years post-master’s experience as a clinical social worker in substance abuse.•        Posses a current, unrestricted licensure as a Clinical Social Worker in any one of the fifty states, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.•        Possess a valid driver’s license and a personal vehicle for use under the contract.BRIEF OVERVIEW OF DUTIESa.        Perform a full range of case management services on b.        As the primary function, provide a comprehensive range of case management services to inpatients, outpatients and their families, with substance abuse, behavioral health and brain injury issues.c.        Maintain resource information for referral to civilian agencies as appropriate.d.        Document case management interventions in accordance with command, directorate, department and professional standards of practice, e.        Plan, coordinate, and provide comprehensive referral services.f.        Function as a member of the Traumatic Stress and Brain Injury multidisciplinary treatment team, presenting reports at team conferences and participating in the evaluation and treatment planning.g.        Plan, coordinate, and provide comprehensive referral services to other patient and family resources.  h.        Implement local strategies using inpatient, outpatient, onsite and telephonic CM; implement policies and protocols for home health assessments and outcome measures.i.        Implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, databases for community resources, etc.j.        Participate in multidisciplinary discharge planning and other professional staff meetings as indicated for complex patient cases and develop a database and knowledge of local community resources.k.        Demonstrated professional knowledge gained through specialized training and experience in the application of case management services.l.        Demonstrated knowledge of domestic violence and substance abuse.
