Loretto is seeking a qualified Nurse Screener (RN) for the CareLink program at St Joseph's Hospital.  The CareLink Representative provides discharge planning services to those patients in need of long-term care.  The CareLink Representative also interacts extensively with Physicians, patients, and family members.DUTIES:    Obtains referrals from Case Managers, Physicians, and other members of the healthcare team for those patients in need of an alternative level of care    Interviews patients, family or significant others to explain the CareLink program, the discharge planning process, and insurance coverage.     Reviews the patient’s medical record in order to determine the appropriate level of care required    Assesses the patient’s physical, social, and financial needs, and makes appropriate referrals    Contacts the patient’s facility choices to screen, and explains CareLink’s expectations of timeliness of screening and their determination of acceptance    Contacts patient or family member with bed offer.  Contacts Physician to complete necessary paperwork.  Arranges for copy of the medical record, arranges for PRI and transportationFor a complete job description and application, please visit www.loretto-cnyjobs.com and review job 4235 REQUIRES:    Registered Nurse (RN) with a minimum of 5 years clinical experience in a hospital setting or long-term care setting    Previous discharge planning, home care, and/or managed care experience desired    Must demonstrate strong organizational, management, and leadership skills, as well as strong oral and written communication skills    Must demonstrate an ability for problem solving and conflict management
